Steps

1. Fill out the form and print it. "Do not sign; you must sign in person at your appointment."

2. Provide proof of U.S. citizenship, valid undamaged passport

4. Provide Photocopies and additional documents.

5. Get a passport photo & do not staple or attach it to the application. For additional requirements, please visit:
